What to Expect
AdvaCare is committed to providing excellent quality of care to its clients in the comfort of their own home. Your first visit will consist of collecting information on your medical history including current and past medical conditions, surgeries, medications, family history, social history and drug allergies to name a few. The Nurse Practitioner will then explore your complaints and perform a detailed physical exam and formulate your medical diagnosis. You and your family member or caregiver involved in the visit will participate in the treatment plan and goals that will be met over the following days/weeks including but not limited to medication refills, referrals to specialist, requirement for Home Health evaluations for Physical and other services, as needed, e.t.c. This initial visit usually lasts about an hour.
The follow up visit(s) are shorter and will ensure you are doing well, and that the goals are being met. We are committed to educating you about your condition(s) to help you manage them and maintain optimum state of health and wellness.
We do not provide Emergency and Urgent Care Services (Please call 911 or go to your nearest ER for urgent, life threatening or emergency symptoms). We cater to insured and uninsured clients. We are HIPPA compliant and maintain patient confidentiality.

House call new patient visit - $200.00
House call follow-up visit - $125.00
We encourage that you ensure your insurance is active if this is your method of payment. We offer concierge services and will accept cash payments if you choose not to bill your insurance company. We do not accept check payments or wire transfers.
