Notice of Privacy Practice

Our Privacy Policy provides information about how your health information may be used and disclosed for treatment, billing and operations in accordance with our policies. It also provides you with information on your rights as a patient and how to access and request a copy of this notice as needed. We understand the importance of protecting your personal information and are committed to collecting, using and disclosing your personal information responsibly.


                                                               How We Collect, Use and Disclose Your Personal Information

AdvaCare HC Practitioners INC is required by law to maintain the privacy of your Protected Health Information (PHI). We must notify you of a breach of unsecured PHI or a compromise to the security of your health information. We must follow the privacy practices described herein. We will not use or share your health information except as described in this notice or by your request in writing that we can. You may revoke such authorization at any time, in writing. Upon revocation of your authorization, we will no longer use or disclose your PHI for the reasons covered by your written authorization.


This notice applies to all health care information contained in the records, created by and received by AdvaCare HC Practitioners INC and tells you about the ways in which we may use and disclose your PHI. This notice applies to all AdvaCare HC Practitioners INC employees, medical staff, contractors, and business associates. We will collect, use and disclose information about you for the following purposes:

  • To provide you treatment and for care coordination. We may disclose your health information to doctors, nurses, technicians, pharmacies, home health agencies, or other healthcare professionals who may be involved in your care.

  • To our business associates who provide services to us to help provide treatment, obtain payment or improve health care operations. For example, a third-party biller.

  • To deliver safe, efficient patient care and to ensure continuous high-quality service

  • To bill and collect payment from you, your insurance company or a third-party payer.

  • To enable us to contact and maintain communication with you such sending reminders, distributing health-care information, and to book and confirm appointments.

  • For teaching and demonstrating purposes on an anonymous basis to help improve your care and train our health care professionals.

  • For public health protection and safety issues such as preventing disease, product recalls, reporting births or deaths, suspected abuse, neglect or domestic violence, preventing or reducing a serious threat to anyone’s health, and preventing the spread of a disease or condition.

  • To comply with all legal, administrative and regulatory requirements

  • For research purposes that meets all federal and state requirements to protect your privacy and subject to a special approval process.


We will not disclose or provide any information about any substance abuse treatment, genetic testing, HIV/AIDs status or mental health treatment unless you provide specific written authorization, or we are otherwise required by law to disclose or provide the information.

 You can download a free copy of our Privacy Policy from our website at or request a copy by contacting AdvaCare HC Practitioners INC office. A copy can also be given to you electronically via email or your patient portal when requested.

                                                                                      Patient Rights

  • The right to request a copy of your health information from AdvaCare HC Practitioners INC.

  • The right to request restrictions on how your health information is used or disclosed but your healthcare providers are not required to agree to this restriction. However, if they agree, they will be bound by the agreement.  In most cases, we will comply with your request except for the following reasons

  • as otherwise required by law, the disclosure is to your health plan and the purpose is related to payment or health care operations.

  • the information is needed to provide emergency treatment

  •  your information pertains solely to health care services for which you have paid in full.

  • The right to request an account of disclosures we make of your PHI for purposes other than treatment, payment or health care operations where an authorization was not required.

  • The right to request for an amendment to your health information if you believe your health information is incorrect or incomplete.

  • Your authorized representative can exercise your rights and make choices about your health information when you are not capable of doing so. We will ensure that the person has the proper authority and can act for you before we take any action.

  • The right to request for confidential communications about your health or medical issues from AdvaCare HC Practitioners INC if such request is reasonable.

  • We will never share your information for the purposes of marketing or the sale of your health information unless you give us written authorization.

Note: All request for amendment and restriction must be in writing. We may charge you a small fee for PHI request. Your request for your PHI may be denied in certain circumstances and we will notify you in writing and you may request that the denial be reviewed. We may deny your request for an amendment. If a denial occurs, you will be notified of the reason for the denial.


 If you believe your privacy rights have been violated, you may file a complaint, in writing, with AdvaCare HC Practitioners INC. You may also file a complaint with the Secretary of the Department of Health and Human Services.                                                                                         Changes to this Notice

AdvaCare HC Practitioners INC reserves the right to change the terms of this Privacy Policy and you may request a copy at any time by contacting the office. The revised or changed notice will be effective for all health information we already have about you as well as any information received and maintained in the future.


By signing the consent section of the Patient General Consent for Treatment Form, you have agreed that you have given your informed consent to the collection, use and/or disclosure of your personal information for the purposes that are listed above. If a new purpose arises for the use and/or disclosure of your personal information, we will seek your approval in advance. Your information may be accessed by regulatory authorities under Texas Medical Board. Our office will not under any conditions supply your insurer with your confidential medical history. In the event this kind of a request is made, we will forward the information directly to you for review, and for your specific consent.  When unusual requests are received, we will contact you for permission to release such information. We may also advise you if such a release is inappropriate. You may withdraw your consent for use or disclosure of your personal information, and we will explain the ramifications of that decision, and the process.
