    1/6 Home Care Services Sick Visits We know that illnesses aren't planned and it can be difficult to get an appointment at the last minute. Our Same Day Sick Visits can help when those unplanned illnesses arrive. We are glad to offer same day appointments for when you are sick or have an semi-urgent problem. We are available every weekday. These appointments are for established patients only, not new patients. These appointments aren’t for every type of problem. They are only for those minor yet urgent problems. Here are of some examples of the types of problems we can treat during our Same Day Sick Visits: Cold and flu symptoms Urinary tract infection Sore throat Rash Earache Cough Please understand that Same Day Sick Visits are for minor, urgent medical needs only, not for long-term chronic issues or preventive services. For example, our Same Day Sick Visits are not for diabetic checkups, medication refills, general exams, pap smears, well child exams, or immunizations. Our Same Day Sick Visits are also not for medical emergencies, like chest pain or shortness of breath. You should go to the emergency room or call 911 if you have a medical emergency. Transitional Care Management These visits are for patients recently discharged from the hospital who need a primary care provider for medication reconciliation, medication refills, continuity of care, assessment for establishment of home health services e.g. physical therapy, occupational therapy, specialist referrals, etc. We aim to maintain your wellness and keep you out of the hospital. Adult Primary Care Services We will function as your primary care provider if you do not have one established. Also, we will schedule regular follow up visits to maintain your overall good health, refill your meds, order labs and educate you on your disease process and chronic conditions.

What to Expect ​ AdvaCare is committed to providing excellent quality of care to its clients in the comfort of their own home. Your first visit will consist of collecting information on your medical history including current and past medical conditions, surgeries, medications, family history, social history and drug allergies to name a few. The Nurse Practitioner will then explore your complaints and perform a detailed physical exam and formulate your medical diagnosis. You and your family member or caregiver involved in the visit will participate in the treatment plan and goals that will be met over the following days/weeks including but not limited to medication refills, referrals to specialist, requirement for Home Health evaluations for Physical and other services, as needed, e.t.c. This initial visit usually lasts about an hour. The follow up visit(s) are shorter and will ensure you are doing well, and that the goals are being met. We are committed to educating you about your condition(s) to help you manage them and maintain optimum state of health and wellness.
